Many companies have the same type of problems that happen over and over again - safety incidents, equipment failures, quality issues, etc. They try different "fixes", but they just don't work. It is important to investigate every incident and near-miss to determine the root causes of problems so that you can fix them once and for all.
Jeff Litzinger and Schuyler Delcamp of Wyoming Machinery Company will cover practices that can be used to identify causal factors in order to fix root causes and develop corrective actions prior to a major issue, without finger pointing and blame.
